The certificate of acknowledgment must be in the form set forth in California Civil Code section 1189. Are you preparing a document and looking to add a California Acknowledgment? How to fill out a California Notary acknowledgement

First line fill in the county in which you are signing in. Send line starts with on that where you enter date of the signing. Before me is the notary name insert your name as it appears on your commission stamp followed by a comma notary public. For example John Do, Notary Public. Personally appeared (name of signer) enter their name as it appears on the document make sure the name is exactly how the signature line reads. Last part sign your name the exact way you signed your oath and affix a clear stamp next to your name.
